Job Overview:

Join our team as a weekend Health Advocate at our virtual heart failure clinic. This role offers a distinctive schedule, with shifts on Friday afternoons and evenings (3pm-9pm EST), as well as full-day shifts on Saturdays and Sundays (9am-9pm EST), totaling 30 hours per weekend. Additionally, you may be required to cover up to one (1)  weekday evening triage shift totaling four (4) hours (Monday to Thursday, 5-9pm EST). To ensure your success, an initial training period will be conducted on weekdays, providing you with the preparation necessary for your new role.

We seek a dedicated and compassionate Health Advocate with a strong medical background, particularly in cardiac care, to join our virtual telehealth-based cardiac clinic. The Health Advocate will be responsible for conducting biweekly calls to patients, providing support, coaching on lifestyle modifications, and ensuring adherence to treatment plans. The ideal candidate will have experience as a medical assistant, possess excellent communication skills, and demonstrate a deep understanding of cardiac health. Additionally, the Health Advocate will play a crucial role in escalating clinical concerns to the team nurse and ensuring patient adherence to remote patient monitoring.


Responsibilities:

  • Patient Engagement:
    • Conduct biweekly outreach calls to patients enrolled in the virtual telehealth-based cardiac clinic.
    • Establish care with patients and provide assistance through introductory steps and registration.
    • Build rapport with patients and provide empathetic support to address their concerns and questions.
    • Communicate with ancillary services and providers such as laboratories and primary care physicians office’s to ensure seamless care is provided to the patient.
  • Medical Education:
    • Coach patients with the assistance of registered nurses on cardiac care, treatment plans, and lifestyle modifications.
    • Ensure patients understand the importance of following prescribed cardiac care guidelines such as remote patient monitoring, sodium consumption, etc. 
    • Conduct medication reconciliation using templated guidance from clinical staff.
  • Treatment Plan Adherence:
    • Monitor and encourage adherence to prescribed treatment plans.
    • Collaborate with healthcare providers to address any barriers to treatment plan compliance.
  • Remote Patient Monitoring:
    • Guide and train patients on the use of remote patient monitoring devices.
    • Ensure patients are consistently adherent to remote monitoring protocols.
  • Escalation to Registered Nurse:
    • Recognize and escalate clinical concerns to the team nurse for further evaluation.
    • Collaborate with the healthcare team to address acute medical issues promptly.
  • Documentation and Reporting:
    • Maintain accurate and up-to-date records of patient interactions and progress.
    • Provide regular reports to the patient’s healthcare team regarding patient engagement, challenges, and escalations.
  • Communication and Collaboration:
    • Collaborate with medical professionals, including cardiologists and other healthcare providers, to address patient needs effectively.
    • Participate in regular team meetings to discuss patient cases and share insights.

Requirements:

  • Education and Experience:
    • Minimum of 2 years of experience as a medical assistant, preferably in cardiac care.
    • Medical Assistant certification is required. 
    • Knowledge of cardiac conditions, treatments, common medications, vitals signs and symptoms related to cardiac conditions.
  • Communication Skills:
    • Excellent verbal and written communication skills.
    • Ability to convey complex information in a clear and understandable manner.
  • Empathy and Compassion:
    • Demonstrate empathy and compassion in patient interactions.
    • Ability to connect with patients on a personal level.
  • Technology Proficiency:
    • Comfortable with virtual telehealth platforms and basic computer skills.
    • Ability to adapt to new technologies and software.
  • Team Player:
    • Ability to work collaboratively with a multidisciplinary healthcare team.
    • Willingness to contribute to a positive and supportive work environment.
