The team acts as a liaison between our provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. This role requires leadership skills, patience and adaptability, a diligent work ethic, excellent communication skills, and the ability to work independently and collaboratively with external partners and internal, cross-functional teams.
