POSITION TITLE: Advanced Practice Clinician (NP/CNM/PA)
LOCATION: Remote within ME/NH/VT
HOURS: Full Time, 30 hours/week, Hourly (Non-Exempt)

POSITION PURPOSE:
As an integral part of health care delivery at PPNNE, the Remote Care Team provides both Telehealth clinical care to patients, as well as Centralized Clinical Support to both patients and health center staff regarding lab work follow up, patient communications, referral care coordination, and health information management Affiliate-wide. The Remote Care Team APC will support PPNNE’s mission of providing accessible reproductive health care through delivering both high quality, patient-centered, personalized telehealth visits, as well as daily integration of remote health care delivery support services to patients and health care teams Affiliate-wide.
YOUR DAY- TO-DAY RESPONSIBILITIES:- Provides exceptional patient-centered care, in a manner that ensures compliance with current PPNNE efficiency and productivity standards.
- Assesses patient's health status and prescribes appropriate contraceptive methods, treatments for GYN problems
- Counsels/educates patients regarding general reproductive health care; all FDA-approved methods of birth control, including emergency contraception; pregnancy options including abortion, adoption as well as Gender Affirming Hormone Therapy
- Provides health care that is culturally and linguistically appropriate to PPNNE patient populations.
- Performs appropriate clinical care via telehealth as well as physical exams, with focus on the reproductive system, including standard initial and annual exams, and screening procedures
- Refers patients to physicians and other health or social services as appropriate
- Responsible for accurately coding for medical services provided, in support of the organization’s overall revenue cycle.
- Remote Care Team APC’s must be willing/able to provide in-person care in their home site health center as needed in addition to providing telehealth care.

COMPENSATION:- Pay Range - for this position is based on years of relevant work experience. Candidates with 0-6 years of experience can expect to earn between $45.46 - $53.64 per hour. As experience increases, so does the rate of pay, with candidates who have 17 or more years of direct experience earning up to $72 per hour.

KNOWLEDGE, SKILLS AND ABILITIES:- Master’s degree and completion of an accredited Nurse Practitioner or Physician Assistant program
- Eligibility for state and DEA licensing as an Advanced Practice Clinician in Vermont, Maine, and New Hampshire

- Broad based clinical GYN knowledge and skills
