<p><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">The Special Education Transition Coordinator is primarily responsible for coordinating and initiating referral for services to Ohio agencies with the assistance of IEP Case Managers and teachers. He or she will conduct bi-weekly or monthly initiatives specific to Transition Services to provide resources and updates to the Special Education staff at all of the ACCEL online schools. The Transition Coordinator will work closely with intervention specialist, families, and students with disabilities to assist in ensuring that the student has the necessary vocational skills experience and education that will enable the student to be a self-supporting citizen, upon graduation.</span></p><p><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><em><strong>Eligibility: Open to residents in Ohio</strong></em></span></p><h2><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><strong>About the Opportunity</strong></span></h2><ul><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Provide IEP Case Managers and families with resources beginning at kindergarten for families with the most intense needs.</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Provide IEP Case Managers the most up-to-date information on transition to life activities and opportunities for students ages 13 and above through monthly Live or Asynchronous updates to share with SWD staff</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Periodically communicate with Opportunities for Ohioans with Disabilities liaison for referrals</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Develop and maintain a database of Age Appropriate Assessments and activities teachers and students can complete in accordance with the students IEP’s.</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Communicate frequently with teachers and parents of students with the highest needs to ensure they have the services they need from Ohio Board of Developmental Disabilities</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Be willing to participate in Professional Development activities provided by Ohio Department of Education</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Focus end-of-year activities on students who are graduating to ensure the necessary services are connected such as through Opportunities for Ohioans with Disabilities or the student’s local board of Developmental Disabilities.</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Provide assistance as needed to provide additional support to IEP Case Managers to ensure compliance with Indicator 13</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Periodically review a prescribed number of IEP’s in accordance with Indicator 13 checklist and provide additional follow up training for compliance&nbsp;</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Serve as IEP Case Manager for students Diploma Deferment program, providing necessary Transition Services and follow-up</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Participate as a member of the Internal Monitoring team</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Perform all other job duties as assigned</span></li></ul><h2><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><strong>About You</strong></span></h2><ul><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Bachelor’s Degree in Special Education, Master’s Degree Preferred</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Transition to Work certification or willingness to work towards obtaining state certification.</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Minimum 3 years of experience providing special education services in an online model</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Current Ohio Special Education Teacher or Administrator Certification</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Proficiency in Microsoft Office and Google Suite Products</span></li><li style="font-family: helvetica, arial, sans-serif; font-size: 12pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 12pt;">Ability to handle confidential information responsibly and exhibit sound judgment while maintaining that confidentiality.</span></li><li style="font-family: helvetica, arial, sans-serif;
