The Role
We are seeking a compassionate and skilled Licensed Clinical Social Worker/Medical Social Worker to join our team. This position involves providing comprehensive psychosocial assessments, counseling services, and care coordination for patients and their families within a virtual medical setting. The ideal candidate will have experience in healthcare environments and a strong commitment to patient-centered care. Experience in Cardiology preferred.
Responsibilities
Clinical Services
- Conduct comprehensive psychosocial assessments to identify patient and family needs
- Develop and implement treatment plans in collaboration with interdisciplinary teams
- Conduct discharge planning and coordinate community resources
Care Coordination
- Collaborate with physicians, nurses, and other healthcare professionals to ensure holistic patient care
- Coordinate referrals to community resources, support services, and specialty care
- Facilitate communication between patients, families, and medical teams
- Participate in interdisciplinary team meetings and case conferences
Patient and Family Support
- Provide emotional support and counseling to patients facing medical diagnoses
- Educate patients and families about medical conditions, treatment options, and coping strategies
- Assist with advance directive planning and end-of-life care discussions
Documentation and Compliance
- Maintain accurate and timely clinical documentation in proprietary technology and/or electronic health records
- Ensure compliance with HIPAA, state regulations, and hospital policies
- Complete required assessments within established timeframes
Qualifications
Education and Licensure
- Master's degree in Social Work (MSW) from a CSWE-accredited program
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent
- Valid state professional license in good standing
Experience
- Minimum two years of clinical social work experience preferred
- Experience in healthcare/medical settings strongly preferred
- Knowledge of medical terminology and healthcare systems
- Experience with electronic health record systems
