This role is focused on developing relationships, facilitating admissions, and being a contributing member of a dynamic and collaborative team!We are currently seeking a healthcare professional to share our vision with hospitals, facilities, and providers and to educate about the amazing benefit of community-focused not profit hospice.Schedule:Monday - Friday; 8:30am - 5:00pmLocation: Sonoma and Napa CountiesBy the Bay Health follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to):<ul><li>Develop relationships, maintain regular contact, and resolve issues with referral sources including physicians, hospitals, nursing facilities, residential care facilities for the elderly, senior housing, and community service agencies to ensure ongoing quality of care and referrals.</li><li>Assist Director of Provider and Community Relations in negotiating or redefining contract provisions as necessary and in accordance with relevant laws and regulations.</li><li>Participate in Annual Contract Review meetings with contracted providers.</li><li>Identify, plan, and implement opportunities for outreach activities including giving presentations, providing informational luncheons, staffing display tables at community events, attending open houses and making regular visits to physician offices.</li><li>Provide regular written and/or oral reports of outreach activities to Director of Provider and Community Relations.</li></ul>Education/Experience:<ul><li>Bachelor’s degree (B.A.) in Marketing, Business Administration or related field from a four-year college or university; or two years related experience and/or training; or equivalent combination of education and experience.</li><li>Minimum two years marketing experience in a health care setting, preferably in hospice care operations.</li></ul>Certificates, Licenses, Registrations:<ul><li>Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.</li></ul>Pay Range:$85K - $118K annually <div class="content-conclusion">By the Bay Health is proud to serve our community as an equal employment opportunity employer.  Everyone is valued and welcome at BTBH.
