Physical Therapy Evaluator - Little Rock

Evaluation availability to be provided over at least 4 days of each week, with open to rescheduling on the 5th day.</li><li>When communication needs to occur with the Family Relations Team or Financial Services, the evaluator will communicate through the Therapy Coordinator/Administrative Coordinator and/or Therapy Director to obtain needed information.</li><li>When provided an evaluation folder, the folder will be reviewed within 48 hours for any additional information needed. The evaluator will contact the family to obtain further information to prepare for evaluation including, but not limited to, needs for feeding evaluation, ortho evaluation, or medical history.</li><li>Evaluate children using appropriate assessment protocols, tools, and observations to follow funding guidelines for each particular child.</li><li>Communicate with family during and/or within 72 hours of the evaluation with the results of the evaluation and recommendations. </li><li>Complete evaluation write-up utilizing Pediatrics Plus template per funding source guidelines.</li><li>Develop and enter goals and objectives in deTASO.</li><li>Enter information into First Connections CDS system (per EI funding). </li><li>Provide a welcoming environment for families.</li><li>Turn in all evaluation write-ups from previous week by noon on the Monday following the initial evaluation period, along with audit forms per funding source. Any corrections are due 24 hours after a report is given back to the evaluator.</li><li>Provide documentation of all therapy services by locking and electronically signing weekly notes as well as efficiency reports.</li><li>Willing to travel to additional sites for assistance in initial evaluations should a need arise, and schedule allows. </li><li>When not able to meet evaluation billing minimum for a week, take the following initiatives:<ul><li>Seek out re-evaluations from discipline team</li><li>Request developmental re-evaluations through site administration </li><li>Assist with coverage board needs</li><li>If evaluations are not available, participate in assigned tasks or projects as directed by leadership. </li></ul></li><li>Collaborate with the primary therapist for re-evaluations in regard to progress, areas of concern, treatment planning and recommendations. Communicate results of evaluation, including changes in treatment recommendations, areas qualified in, and/or discharge from services.</li><li>Submit all time off requests to the director for approval.
