This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Patient Navigation Program Advisor, Continuity based in United States.

This role brings hands-on patient navigation expertise into the design and delivery of an AI-powered radiology follow-up platform. You will help healthcare organizations build effective programs that ensure actionable findings move from radiology reports into completed patient care. Working closely with implementation, customer success, clinical, and product teams, you will shape workflows, training, configuration, and product priorities. Your experience will help distinguish tasks that require clinical judgment from those that can be safely automated. The role combines customer partnership, clinical expertise, product influence, and program development in a fast-moving healthcare technology environment. It offers an opportunity to improve follow-up processes while helping make high-quality patient navigation more accessible across diverse healthcare organizations.


Accountabilities:
  • Partner with implementation and customer success teams to design patient follow-up programs that align with each customer's staffing model, referral patterns, workflows, and EHR environment.
  • Advise customers and internal teams on configuration decisions, including worklist structures, patient communication templates, outreach cadences, and resolution categories.
  • Support navigator training and user acceptance testing to ensure teams are prepared to use the platform effectively before each customer launch.
  • Serve as a clinical and operational resource for customer navigation teams, helping answer questions and resolve workflow challenges during the initial months following implementation.
  • Represent the patient navigator perspective in product requirements, design reviews, feature prioritization, and ongoing improvements to the user experience.
  • Collaborate with clinical stakeholders to review follow-up recommendations and categorization, helping ensure workflows reflect real-world radiology navigation practices.
  • Help identify which follow-up activities require clinical judgment and which can be appropriately supported or automated through software and administrative workflows.
  • Translate practical patient navigation expertise into scalable training materials, operational playbooks, success benchmarks, and best practices that can be adopted across customer programs.

Requirements:

  • Bring at least 2 years of hands-on experience in radiology follow-up, patient navigation, care coordination, or a comparable healthcare role such as nurse navigator, radiology technologist, or care coordinator.
  • Demonstrated experience designing or operating a patient follow-up program and leading, supporting, or training the staff responsible for delivering it.
  • Strong practical experience managing a follow-up worklist and tracking patients through to completed follow-up, including complex cases involving unreachable patients, incorrect referring providers, missing orders, and other operational barriers.
  • Comfortable working across electronic health record systems such as Epic, Cerner, Meditech, or similar platforms, with a practical understanding of how orders, results, referrals, and patient communications move through healthcare organizations.
  • Willingness to provide direct, specific, and actionable product feedback while advocating strongly for the needs and experience of frontline users.
  • Self-directed and comfortable operating in an environment with ambiguity, evolving priorities, and a small team moving quickly to solve meaningful healthcare problems.
  • Available to participate in customer-facing meetings, training sessions, and implementation activities during standard US business hours.
  • A clinical credential such as RN or RT(R), whether currently active or previously held, is a strong plus.
  • Experience working within a radiology department or imaging center is preferred, as is familiarity with radiology follow-up guidelines such as Fleischner, TI-RADS, LI-RADS, or Lung-RADS.
  • Experience managing or training other navigators, supporting Lung Cancer Screening programs, working with registry reporting such as ACR NRDR, or participating in health IT implementations is advantageous.
