At Rely Health, we’re on a mission to transform the way patients navigate care after leaving the hospital. Too often, patients and families are left overwhelmed—unsure of which doctors to see, what order to schedule appointments, or how to manage the costs. This leads to gaps in care, missed follow-ups, and unnecessary ER visits.

With our AI-powered patient navigation platform and compassionate team of navigators, we simplify the care journey. We help patients book follow-up appointments, overcome barriers like insurance challenges or transportation, and connect with the right providers at the right time. Together, we’re creating a more connected, supportive, and healthier future for communities across the country.

We’re a fast-growing healthcare startup backed by mission-driven investors and led by entrepreneurs and healthcare leaders who have built and scaled successful organizations. If you’re energized by messy problems, scaling solutions, and leading with impact, we’d love to meet you.

About the Role

We’re looking for someone passionate about operations to lead and scale our growing care navigation team. Depending on experience, this could be a Director or VP of Operations. This is a rare opportunity to join the leadership team of a healthcare startup that is rapidly scaling.

The ideal candidate is a startup operator who thrives on building systems, designing workflows, mentoring teams, and turning complexity into clarity. You’ll collaborate closely with our product and customer success teams.

Responsibilities

  • Lead and scale day-to-day operations. Oversee scheduling, training, quality assurance, compliance, and team management for both onsite and virtual Patient Care Navigators.
  • Design and optimize workflows. Build evidence-based, tech-enabled processes that improve patient outcomes, reduce ER bounce-backs, and meet client contract metrics.
  • Develop tools and dashboards. Partner with Product to implement systems that track KPIs, forecast staffing needs, and improve team efficiency.
  • Drive cross-functional collaboration. Work with Product, Sales, and Client Success teams to ensure operations support growth and client satisfaction.
  • Ensure compliance. Maintain HIPAA standards, implement operational best practices, and prepare the team for audits or regulatory reviews.
  • Scale the organization. Anticipate future needs, recruit and develop managers, and build structures that support long-term growth.

Requirements

  • 7–10+ years in operations, with at least 3 years in a leadership role (healthcare startup experience strongly preferred).
  • Proven success in scaling operations, managing budgets, and leading multi-site or hybrid teams.
  • Strong data analysis, workflow design, and process improvement skills.
  • Comfortable working in fast-moving, ambiguous startup environments.
  • Excellent communication and leadership skills—able to inspire, coach, and hold teams accountable.

Bonus Qualifications:

  • Experience in call center operations, patient navigation, or population health management.
  • Background in scaling tech-enabled healthcare services.
  • Familiarity with enterprise-level contracts and client success management

Competencies (Knowledge/Skills/Abilities):

  • Ability to build relationships across the company to effectively drive operations
  • Strong analytical skills
  • Strong communication and teamwork skills, with the ability to collaborate effectively with cross-functional teams.
  • Ability to lead a team with a servant leadership mindset
