Karoo is seeking to add an important care team member, the Karoo Koach.  This individual will provide health coaching and coordination to members with cardiovascular prevalent diseases.  

Responsibilities

  • Determines patient eligibility for Karoo Health wraparound services 
  • Operate within practice setting(s) and coordinate across multiple team members 
  • Initial patient onboarding/activation through program and technology education 
  • Completes ongoing care plan education and monitoring of adherence to the care plan
  • Coordinates health coaching as directed by the Cardiologist or other qualified health professional that address goals identified by the care plan
  • Implements a coaching plan to activate the patient and address barriers to change (e.g. medication adherence, chronic disease education, life-style changes, etc.) 
  • Coordinates with the Cardiology practice to provide insight into the patient’s journey and clinical issues
  • Coordinates patients to community and health plan service programs
  • Monitors incoming clinical submissions from wearable devices 
  • Regularly interacts with practice staff to provide line-of-sight into patient admissions, readmissions, and ED visits
  • Are comfortable and have demonstrated experience with using multiple technological tools in your day-to-day; you are able to adapt and grow with technological developments quickly
  • Special projects, initiatives, and other job duties as assigned 

Qualifications

  • BA/BS in Health Education, Exercise Physiology, Health Sciences, Human Services 
  • Minimum 3 years of related experience in health education, health coaching, patient education
  • Certification, advanced certification, and/or license appropriate to the field of specialty (National Board of Health and Wellness coaching)
  • Experience with triage and working within a team-based care model in an onsite or virtual setting 
  • Experience with team collaboration and coordination of care with external sources
  • Strong interpersonal skills and ability to quickly establish rapport with patients over the phone
  • Inspires patient motivation and enthusiasm for change and/or follow through on care plans and goals
  • Comfortable with ambiguity and a fast-paced culture: You can adapt and approach problems with a solution-oriented mindset 
  • The ability to utilize data to make informed decisions
