Location: Remote (Work from Anywhere)
Schedule: Full-Time
Pay Rate: $16.00 - $17.00 per hour

Overview

Chronic Care Staffing is seeking a proactive and solutions-oriented Follow-up Coordinator to help ensure patients successfully connect with the specialized care they need. In this role, you'll track referrals after submission, communicate with specialist offices, troubleshoot scheduling barriers, and help patients navigate the next steps in their healthcare journey.

This position is ideal for someone who enjoys problem-solving, phone-based coordination, and making a meaningful impact on patient outcomes.

Responsibilities

  • Track and monitor pending patient referrals after submission.
  • Contact specialist offices via phone, fax, and secure portals to verify referral status.
  • Follow up on outstanding referrals and appointment requests.
  • Investigate scheduling obstacles and referral delays.
  • Identify alternative providers when specialists are unavailable or unable to accept patients.
  • Communicate updates and next steps to patients as needed.
  • Utilize internal resource guides to locate appropriate specialists.
  • Maintain accurate records and documentation within internal systems.
  • Collaborate with Scheduling Coordinators and other team members to ensure workflow efficiency.
  • Cross-train across multiple coordination functions to support team needs.

Requirements

Must Have

  • Previous experience in:
    • Customer service
    • Call center operations
    • Administrative support
    • Medical reception
    • Healthcare coordination
  • Strong verbal communication and phone skills.
  • Excellent organizational and multitasking abilities.
  • Ability to work independently and manage follow-up activities without constant supervision.
  • Strong problem-solving mindset and attention to detail.
  • Comfortable learning new systems and processes.
  • Reliable internet connection and ability to work remotely.

Nice to Have

  • Experience working in a medical office or healthcare environment.
  • Familiarity with referral management or appointment scheduling.
  • Experience communicating with healthcare providers and clinics.
  • Understanding of insurance-related scheduling challenges.
  • Knowledge of healthcare administrative workflows.
