We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.</div>The primary location of this position is remote with a schedule of Monday-Friday 9:00am-6:00pm Eastern Time or 8:00am-5:00pm Central Time.What You’ll DoAs part of our Transitions of Care Team, the Community Health Navigator assists patients in getting post-hospital care in the form of telephonic and in-person patient support. In this role, you will:<ul><li style="font-size: 12pt;">Work as part of an interdisciplinary, collaborative team to ensure we are meeting patient needs</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Outreach to patients discharged from emergency rooms to ensure continuity of care and assist with identifying and meeting needs related to social determinants of health</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Deliver resources to families within your geographic area</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Coordinate with facility discharge planners to ensure that there are no barriers to patients obtaining after-hospital resources and care</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Attend daily huddles with your team to communicate patient needs and progress</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Liaise with hospitals and other health systems to make staff aware of Imagine Pediatrics program and support</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Support patients with any in person/community needs related to care coordination/discharge support. These could include, but are not limited to, pharmacy fills, PCP coordination, SDOH needs, and home visits</li><li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Perform other duties as assigned</li></ul>What You Bring & How You QualifyFirst and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need:   <ul><li style="font-size: 12pt;">A bachelor’s degree required</li><li style="font-size: 12pt;">Bilingual (Spanish) required</li><li style="font-size: 12pt;">National CHW certification preferred</li><li style="font-size: 12pt;">CPR certification preferred</li><li style="font-size: 12pt;">3+ years of experience in a healthcare setting (patient-facing hospital or facility role)<ul><li style="font-size: 12pt;">ER setting preferred</li></ul></li></ul>What We Offer (Benefits + Perks) The role offers an hourly range of $23-28 per hour in addition to an annual bonus incentive, competitive company benefits package, and eligibility to participate in an employee equity purchase program (as applicable).
