About the Job

Care coordination is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the individual's health needs through communication and available resources to promote cost-effective outcomes, using evidence-based guidelines as available in daily practice. The Care Coordinator I and II focuses on effective, safe, timely and complete transitions of care. In collaboration with the multidisciplinary healthcare team, this position is responsible for the timely coordination of health care services to meet the patient's individual needs and promote quality and cost effective outcomes.

Medical Assistant Certification is required.

This is a contract position.

Key Functions & Responsibilities (included But Not Limited To):

Care Coordinator I

  • Deliver high-quality remote support via chat, phone, and email, serving as one of the first responders to any inbound questions or requests priority items get a proper response

  • Booking appointments for patients/providers

  • Working collaboratively with providers and practice staff to care for patient

  • Review and update patient insurance packages

  • Collect patient payments over the phone when required

  • Proactive outreach to patients to support lab/image/referral tracking, gaps in care and other quality measure improvement efforts.

  • Effective and compassionate communication with patients and their significant others to plan, implement and follow up on gaps in care.

  • Coordination with other members of the care team, Providers on the care team and consultants to close gaps in care and update EMR.

  • Acts as a liaison between patients, primary care practices and population health and Quality teams in the provision of care.

  • Facilitates partnerships with community partners and resources to assist patients in closing gaps in care

  • Works as part of the interdisciplinary team to meet the goals of the Practice, the provider and the patient. Will support the population health and transformation initiatives as assigned.

  • Provides quality customer service- follows HIPAA rules/regulations.

  • As a member of the patient care team, documents in the EMR, the patient interactions and care planning activities as directed by the Provider and care team.

  • Identifies and appropriately escalates patient related issues to the Provider when required.

  • Manage assigned report tasks and practice-related functions as needed

  • Engage Practice Leadership in a timely manner when providers fail to complete coding and billing information at the end of each visit

  • Facilitate care and streamline handoffs with other parties in the healthcare system, ensuring that our members receive the highest quality care

  • Iterate constantly, leveraging user feedback and your operational experience to improve on what we’ve built

  • Create loyalty among users by contributing to the overall Visana experience

  • Work collaboratively with a team of peers to solve customer issues and complete clinical and administrative tasks on behalf of Visana’s doctors and nurse practitioners

Qualifications

  • 2-3 years of healthcare customer service experience with hybrid practice models

  • Medical Assistant certification REQUIRED

  • Must be willing to work (nights or weekends)
